Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our Care Management team. As a Registered Nurse, you will provide direct patient care, perform nursing assessments, and collaborate with other healthcare professionals to ensure the highest quality of care for our patients. This is a perdiem position.
Responsibilities:
- Perform nursing assessments in the home.
- Provide home safety evaluations.
- Conduct diagnostic evaluations and develop individualized care plans.
- Provide medication management.
- Provide advocacy.
- Accompany clients to medical appointments.
Requirements:
- Valid RN license in [CT]
- Proficient in navigating the community health process.
- Strong knowledge visiting nursing.
- Experience in diagnostic evaluation and developing care plans
- Experience with basic computer skills.
- Experience researching ways to enhance client's quality of life.
Pay: $70.00 - $80.00 per hour
Expected hours: 2 – 20 per week
